Part A costs
Most people pay no premium for Part A. The cost comes when you're hospitalized: a $1,736 deductible per benefit period in 2026, then $0 for days 1–60, $434 a day for days 61–90, and $868 a day for lifetime reserve days. Skilled nursing facility care is $0 for days 1–20 and $217 a day for days 21–100 after a qualifying stay.
Part B costs
The standard Part B premium is $202.90 a month, with a $283 annual deductible. After the deductible, you typically pay 20% of the Medicare-approved amount for covered services, with no annual cap under Original Medicare alone.
Part D costs
Part D premiums vary by plan. What's new and important for 2026: your out-of-pocket spending on covered drugs is capped at $2,100, the old coverage gap ("donut hole") is gone, and a plan's deductible can be no higher than $615. You can also spread costs over the year through the Medicare Prescription Payment Plan.
Medicare Advantage costs
Many Medicare Advantage plans have a low or $0 monthly premium — though you still pay your Part B premium. Instead of coinsurance you usually pay set copays, and the plan caps your in-network medical costs at no more than $9,250 in 2026. Drugs and extras like dental or vision are often included.
Medicare Supplement costs
A Medicare Supplement adds a monthly premium that varies by plan letter, age, location, and insurer. In exchange, it covers most of Original Medicare's deductibles and coinsurance, making your costs low and predictable. Plan F is closed to people who became eligible on or after January 1, 2020; Plan G and Plan N are the popular choices for newer enrollees.
Income surcharge (IRMAA)
If your income is above $109,000 (individual) or $218,000 (joint), you'll pay an income-related surcharge on top of your Part B and Part D premiums, based on your tax return from two years earlier. The surcharge steps up in tiers and the brackets are cliffs. The estimator below shows your exact 2026 amount.
